Medications for School Use



Any medication that a student is to take at school requires a doctor's order.  This includes over the counter medication (for example: ibuprofen, acetaminophen, benadryl), prescribed medications, and/or inhalers.  Take the attached form to the student's physician to be filled out.  The parent/guardian then signs and dates on the bottom of the form.  An adult must bring the medication and authorization form to the school.  Students are not allowed to transport medications to the health office.  

The licensed school nurse will notify the parent/guardian when a refill of the medication is needed at the school.  Please bring the refill to school in a timely manner so the student won't miss any doses while at school.
